Financial Policy Summary
PAYMENT FOR SERVICES IS DUE AT THE TIME OF TREATMENT UNLESS PRIOR ARRANGEMENTS HAVE BEEN MADE.

We will be happy to complete dental insurance forms for our patients and file them for you. There will be a nominal processing fee of $12.00 for each claim that we file. We will only file a claim after we have given you an opportunity to call your dental insurance to find out about your benefits. Please call your dental insurance for the following information:
- Your annual maximum benefits
- The percentage covered for basic and major procedures
- Your deductible and how much of it has been paid
- If there is a waiting period for specific procedures
- If there are limitations for specific procedures
Insurance reimbursement generally takes 3-5 weeks. Your insurance will send payment directly to you unless you sign the "assignment of benefits." If you decide to sign over your benefits, this only means that your insurance check will come directly to our office. Regardless of who receives the check, the payment from insurance will be posted to your account. Please be aware of your deductible, annual maximum and percentage paid by your insurance.
THE PATIENT IS ULTIMATELY RESPONSIBLE FOR PAYMENT IN THE EVENT THE INSURANCE COMPANY DENIES THE CLAIM.
We accept cash, checks, Visa and MasterCard. All credit card payments must include the 3-digit security code on the back of the card and the billing address. Without this information, we are unable to process your payment and treatment will be delayed.
A courtesy call or email will be made to remind you of your next appointment. If you must change an appointment, we require at least 24 hours notice. We reserve the right to charge a $50.00 cancellation fee for less than 24 hours notice. Appointments are an important part of your treatment, and time has been reserved specifically for you. Please be respectful of the practice's and the doctor's time.
